This is the 2021 CKD-EPI creatinine equation, the current standard recommended by the National Kidney Foundation and American Society of Nephrology after the 2020 review of race in eGFR calculations.

The min/max split creates two smooth curve segments around the typical creatinine value for each sex (κ), rather than a single formula across the whole range.

Worked example

50-year-old woman, creatinine 0.9 mg/dL

  1. 1.Scr/κ = 0.9 ÷ 0.7 = 1.286, so min(1.286, 1) = 1 and max(1.286, 1) = 1.286.
  2. 2.1^−0.241 = 1. 1.286^−1.2 ≈ 0.7397.

What glomerular filtration rate measures

The kidneys filter waste products and excess fluid out of the blood through roughly a million tiny filtering units in each kidney, called nephrons. The glomerular filtration rate is the volume of blood the kidneys filter per minute, and it is the single most widely used summary number for how well the kidneys are performing that core function — the higher the rate, generally, the more effectively the kidneys are filtering.

Why GFR is estimated rather than measured directly

Measuring true GFR directly requires injecting a marker substance — such as inulin or iohexol — into the bloodstream and precisely tracking how quickly it is cleared, a process accurate enough for research but too involved for routine use. In everyday clinical practice, GFR is instead estimated ("eGFR") from a routine blood test measuring creatinine, a waste product generated at a fairly steady rate by muscle tissue and normally cleared by the kidneys almost entirely. As kidney filtering capacity declines, creatinine builds up in the blood, and equations like the one this calculator uses convert that single creatinine value into an estimated filtration rate.

Why age and sex are part of the calculation

Creatinine production is loosely tied to muscle mass, which differs on average by sex and tends to decline somewhat with age. Two people can have identical creatinine readings and quite different actual kidney function if their expected creatinine production differs — the equation’s age and sex terms are a population-level correction for that, not an assumption about any individual’s health. Estimating equations have been revised over time as better data on these relationships becomes available; the equation used here is the CKD-EPI 2021 version, the current standard recommended by the major US kidney-health professional bodies.

GFR as a screening measure, not a diagnosis

A single eGFR result is a screening number, not a diagnosis on its own. Creatinine — and therefore the eGFR calculated from it — can shift temporarily with dehydration, acute illness, recent heavy exercise, or diet, which is why standard clinical practice for identifying chronic kidney disease relies on a reduced eGFR (or other evidence of kidney damage) confirmed across at least two tests separated by three or more months, rather than a single reading. eGFR is also typically interpreted alongside other markers, such as the amount of protein in urine (albuminuria), particularly in the normal-to-mildly-reduced range where eGFR alone is a less complete picture.

What this assumes, and where it stops

Assumptions

  • The creatinine measurement is stable — a single recent, non-acute reading. Acute illness, dehydration or recent heavy exercise can temporarily distort creatinine.

Limitations

  • Less accurate at extremes of muscle mass — bodybuilders, amputees and people with muscle-wasting conditions can get misleading results because creatinine production depends on muscle mass.
  • Not validated for children — paediatric kidney function uses separate equations (such as the bedside Schwartz equation).
  • A single result cannot diagnose chronic kidney disease on its own; sustained results over time and other markers are needed.

Common questions

What is a normal eGFR?

Above 90 mL/min/1.73m² is considered normal (stage G1), though eGFR also naturally declines somewhat with age. Between 60 and 89 (stage G2) is common in older adults and is not necessarily a problem on its own — it becomes more significant when combined with other signs of kidney damage, such as protein in the urine.

Why was race removed from the eGFR equation?

Older equations added a fixed multiplier for patients identified as Black, based on average differences observed in the original study populations. A 2021 national task force found this coefficient did not reliably reflect an individual's actual physiology and could delay Black patients' access to kidney disease diagnosis and transplant referral, so the race-free 2021 CKD-EPI equation is now the recommended standard.
